We offer certified Community Health Worker (CHW) and Peer Support Specialist (PSS) services designed to help people overcome barriers, improve wellness, and build healthier, more stable lives.


Our Community Health Workers and Peer Support Specialists meet people where they are and provide support that promotes hope, healing, self-sufficiency, and long-term success.

Community Health Worker

Our Community Health Worker services help individuals connect to the care, resources, and
support they need to improve their overall health and well-being. We work closely with clients to
address social, emotional, and practical challenges that may affect their quality of life.

Services may include:

  • Health education and wellness support
  • Care coordination and service navigation
  • Assistance accessing medical and behavioral health services
  • Referrals to community resources
  • Support with scheduling appointments and follow-up care
  • Medication education and adherence support
  • Advocacy for client needs and goals
  • Help reducing barriers to healthcare access
  • Family support and resource connection
  • Assistance with housing, food, transportation, and other basic needs resources

Peer Support Specialist Services

Our certified Peer Support Specialist services are led by individuals with lived experience who
understand the challenges of recovery, mental health, and life transitions. Peer support provides
encouragement, mentorship, and guidance in a nonjudgmental and empowering way.

Services may include:

  • One-on-one peer support
  • Recovery coaching and encouragement
  • Goal-setting and personal empowerment skills
  • Help developing coping and life skills
  • Support with community reintegration
  • Guidance in navigating recovery and support system
  • Mentoring based on lived experience
  • Emotional support and encouragement
  • Connection to recovery resources and community programs
  • Advocacy and self-advocacy skill-building
  • Support with building healthy routines and structures
  • Crisis prevention support and wellness planning
